Safe Care Connection offers a range of in-home and community-based services designed to support individuals with varying levels of need. From non-medical assistance and companionship to skilled nursing and habilitative support, our services are delivered with a focus on dignity, consistency, and person-centered care. Each service is provided through coordinated referral pathways and tailored to align with individual goals, routines, and care plans.

ODP Services Client
In-Home Care
In-home care services provide non-medical support for individuals who wish to remain safely and comfortably in their own homes. These services include assistance with activities of daily living such as personal hygiene, mobility, meal preparation, and routine household tasks. In-home care is designed to support independence while offering consistent, respectful assistance tailored to each individual’s needs, preferences, and daily routines.


Respite Care
Respite care offers temporary support for individuals while providing relief for family members and primary caregivers. This service ensures continuity of care in a familiar environment, allowing caregivers time to rest, attend appointments, or manage other responsibilities. Respite services are delivered with the same attention to consistency, safety, and individual comfort as ongoing care.
ODP Services
We provide Office of Developmental Programs (ODP) services through Pennsylvania’s Home and Community-Based Services waivers. These services support children and individuals with intellectual and developmental disabilities (including autism) by offering habilitative care, respite support, nursing, and behavioral health services. ODP services are designed to promote independence, participation in the community, and improved quality of life through individualized, person-centered care planning.


Skilled Nursing
Skilled nursing services are provided by licensed healthcare professionals and support individuals with medical or clinical needs in the home or community setting. Services may include health monitoring, medication support, coordination with physicians, and ongoing clinical assessments. Skilled nursing care is delivered with attention to safety, accuracy, and collaboration with broader healthcare teams.
Companionship
Companionship services provide social connection and supportive presence for individuals who do not require hands-on care but may benefit from regular interaction. Services focus on conversation, shared activities, and meaningful engagement that supports emotional well-being and reduces feelings of isolation. Companionship is centered on respect, independence, and comfort, allowing individuals to remain socially connected while maintaining their routines.


Habilitative Care
Habilitative care supports individuals in developing and maintaining essential life skills needed for daily living and community participation. Services may focus on communication, self-care, social interaction, and routine-building, depending on individual goals. Habilitative care is structured, supportive, and person-centered, helping individuals build confidence and independence over time.
